Type 2 diabetes mellitus (non-insulin dependent) to improve blood sugar control, usually along with diet and exercise; may be used alone or with other antidiabetic medicines
Gliclazide is a sulfonylurea that stimulates insulin release from pancreatic beta cells by closing ATP-sensitive potassium channels. This increases insulin secretion and helps lower blood glucose, particularly after meals. The modified-release formulation provides a gradual effect over the day.
Oral tablet (modified-release). Usually taken once daily with breakfast; swallow whole with water—do not crush, split, or chew. Take at the same time daily; dose is individualized by the doctor based on blood glucose/HbA1c.
Hypoglycaemia (low blood sugar), nausea, stomach pain, diarrhoea, constipation, headache, dizziness, weight gain, skin rash/itching, increased liver enzymes
Risk of hypoglycaemia—more likely with missed meals, excessive exercise, alcohol intake, elderly, kidney/liver impairment, or when combined with other antidiabetics. Not for type 1 diabetes or diabetic ketoacidosis. Use caution in renal/hepatic disease; monitor blood glucose and HbA1c regularly. Stop and seek care if severe allergy, jaundice, or unusual bleeding/bruising occurs. During illness, surgery, or reduced intake, dose adjustment may be needed; do not change dose without medical advice.
Other antidiabetics/insulin (increased hypoglycaemia risk), alcohol (can increase hypoglycaemia), beta-blockers (may mask hypoglycaemia symptoms), NSAIDs/salicylates (may enhance hypoglycaemia), azole antifungals e.g. fluconazole/miconazole (may increase effect), sulfonamides (may increase hypoglycaemia), corticosteroids (raise glucose), thiazide/loop diuretics (raise glucose), rifampicin (may reduce effect), warfarin (monitor INR)
