Pregnancy
Glimibrit M 1mg/500mg Tablet may be unsafe to use during pregnancy.<br>Animal studies have shown adverse effects on the foetus, however, there are limited human studies. The benefits from use in pregnant women may be acceptable despite the risk. Please consult your doctor.
Breastfeeding
Glimibrit M 1mg/500mg Tablet is probably safe to use during lactation. Limited human data suggests that the drug does not represent a significant risk to the baby.
Type 2 diabetes mellitus in adults when diet and exercise alone are insufficient; useful when fasting and post-meal glucose are both elevated requiring dual therapy.
Common: hypoglycaemia, nausea, diarrhoea, metallic taste, weight gain, headache; Serious: lactic acidosis (metformin), severe/prolonged hypoglycaemia, cholestatic jaundice, blood dyscrasias.
Glimepiride closes pancreatic β-cell ATP-sensitive K+ channels (SUR1/Kir6.2), causing depolarisation, Ca2+ influx and insulin release; metformin activates AMPK to reduce hepatic gluconeogenesis and improve peripheral insulin sensitivity.

Alcohol

Kidney
Glimibrit M 1mg/500mg Tablet should be used with caution in patients with kidney disease. Dose adjustment of Glimibrit M 1mg/500mg Tablet may be needed. Please consult your doctor.<br>Use of Glimibrit M 1mg/500mg Tablet is, however, not recommended in patients with severe kidney disease. Regular monitoring of kidney function test is advisable while you are taking this medicine.
Liver
Glimibrit M 1mg/500mg Tablet should be used with caution in patients with liver disease. Dose adjustment of Glimibrit M 1mg/500mg Tablet may be needed. Please consult your doctor.<br>Glimibrit M 1mg/500mg Tablet is generally started with low dose in patients with mild to moderate liver disease and its use is not recommended in patients with severe liver disease.

Monitor renal function before and during therapy; stop and seek urgent care for rapid breathing, severe weakness, abdominal pain (lactic acidosis symptoms); counsel on hypoglycaemia recognition and carrying glucose; dose reduction needed in elderly or reduced renal function; avoid dehydration and prolonged fasting; use caution in G6PD deficiency (haemolysis risk with sulfonylureas).
Alcohol: increases hypoglycaemia and lactic acidosis risk; iodinated contrast media: precipitates metformin-associated lactic acidosis—hold metformin at/around procedure per protocol; beta-blockers (propranolol/metoprolol): mask hypoglycaemia symptoms; fluoroquinolones (levofloxacin): dysglycaemia; ACE inhibitors: may enhance glucose-lowering effect; sulfonamides/azoles: increase sulfonylurea levels via CYP inhibition.
