Neuropathic (nerve) pain such as diabetic neuropathy, post-herpetic neuralgia, sciatica/radiculopathy, neuropathic pain associated with spinal disorders; also used when nerve pain has burning, tingling or shooting character.
Pregabalin reduces abnormal nerve signaling by binding to the alpha-2-delta subunit of voltage-gated calcium channels, decreasing release of pain neurotransmitters. Nortriptyline is a tricyclic antidepressant that increases norepinephrine/serotonin in pain pathways and modulates nerve pain. Together they provide enhanced relief in chronic neuropathic pain.
Tablet; take exactly as prescribed, usually once daily at night (may cause drowsiness). Can be taken with or without food; swallow whole with water. Do not stop suddenly—dose should be tapered by a doctor.
Dizziness, drowsiness/somnolence, dry mouth, constipation, blurred vision, weight gain, peripheral edema (swelling), difficulty concentrating, fatigue
May cause sleepiness, dizziness and blurred vision—avoid driving/operating machinery until effects are known. Use caution in elderly, patients with kidney disease (pregabalin dose adjustment), glaucoma, urinary retention, heart disease/arrhythmias, constipation, and in those with history of substance misuse. Risk of suicidal thoughts/behavior with antiepileptics; seek help for mood changes. Nortriptyline can cause anticholinergic effects and may affect heart rhythm; overdose can be serious. Do not stop abruptly to avoid withdrawal symptoms.
Alcohol and other CNS depressants (opioids, benzodiazepines, antihistamines) increase sedation/respiratory depression; MAO inhibitors (contraindicated/avoid); other antidepressants/serotonergic drugs (risk of serotonin syndrome); anticholinergics (additive dry mouth/constipation/urinary retention); QT-prolonging drugs/antiarrhythmics (arrhythmia risk); tramadol (seizure/serotonin risk); gabapentin (additive dizziness/sedation).
