Treatment of type 2 diabetes mellitus to improve blood sugar control along with diet and exercise; used when metformin alone is insufficient or as combination therapy.
Sitagliptin is a DPP-4 inhibitor that increases incretin hormones, leading to increased insulin release and decreased glucagon secretion in a glucose-dependent manner. Metformin decreases hepatic glucose production and improves insulin sensitivity, helping cells use glucose better.
Tablet for oral use. Take exactly as prescribed by your doctor, usually with meals to reduce stomach upset. Swallow whole with water; do not crush/chew if it is an extended-release formulation (verify pack). Regularly monitor blood glucose/HbA1c.
Nausea, vomiting, diarrhea, abdominal pain, loss of appetite, headache, upper respiratory tract infection, sore throat, hypoglycaemia (especially with insulin/sulfonylureas), flatulence
Type 2 diabetes only (not for type 1 diabetes or diabetic ketoacidosis). Metformin may rarely cause lactic acidosis—risk increases with kidney disease, severe dehydration, sepsis, heavy alcohol use, or hypoxia; kidney function should be checked before and during treatment. Sitagliptin has been associated rarely with pancreatitis and severe joint pain; seek medical help for severe abdominal pain. Use caution in renal impairment; dose adjustment/avoidance may be required depending on kidney function. Stop temporarily before iodinated contrast imaging and major surgery as advised by doctor.
Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cimetidine, diuretics and ACE inhibitors/ARBs (may affect renal function), insulin and sulfonylureas (increased hypoglycaemia risk), digoxin (sitagliptin may increase levels)
