Tapentadol (100mg)
Management of moderate to severe chronic pain where continuous, around-the-clock opioid analgesia is needed (e.g., chronic musculoskeletal pain, neuropathic pain) when alternative treatments are inadequate.
Tapentadol is a centrally acting opioid analgesic. It works by activating mu-opioid receptors to reduce pain perception and by inhibiting norepinephrine reuptake, strengthening descending pain inhibition pathways.
Take orally as prescribed, usually every 12 hours for extended-release tablets. Swallow whole with water; do not crush, chew, or break. Can be taken with or without food; take at the same times each day.
Nausea, vomiting, constipation, dizziness, drowsiness/somnolence, headache, dry mouth, fatigue, sweating, itching
Prescription-only opioid; risk of misuse, addiction, and dependence. May cause life-threatening respiratory depression, especially at start or after dose increases; avoid in severe asthma/respiratory compromise. Do not use in paralytic ileus; caution in head injury/raised intracranial pressure, seizures, liver or kidney impairment, hypotension, elderly, and patients with sleep apnea. May impair driving/operating machinery. Do not stop suddenly after long-term use (withdrawal); taper under medical supervision.
Alcohol and other CNS depressants (benzodiazepines, sedative-hypnotics, antipsychotics, antihistamines) increase sedation/respiratory depression; MAO inhibitors (contraindicated/avoid within 14 days); serotonergic drugs (SSRIs/SNRIs/TCAs/triptans/linezolid) risk serotonin syndrome; other opioids; mixed agonist/antagonist opioids (e.g., buprenorphine, nalbuphine) may reduce analgesia/precipitate withdrawal; strong CYP inducers may reduce effect; antihypertensives may increase hypotension.
