Treatment of hypertension (high blood pressure); helps reduce risk of cardiovascular events associated with uncontrolled BP

Cilnidipine is a calcium channel blocker that relaxes blood vessels by reducing calcium entry into vascular smooth muscle (L- and N-type channel blockade). Metoprolol succinate is a cardioselective beta-1 blocker that lowers heart rate and reduces the force of contraction, decreasing blood pressure and cardiac workload.

Take orally as advised by the physician, preferably at the same time each day. Swallow the XL/ER tablet whole with water; do not crush or chew. Can be taken with or without food; if it upsets the stomach, take with food.

Use with caution in asthma/COPD (beta-blocker may worsen bronchospasm), diabetes (may mask hypoglycaemia symptoms), thyroid disorders, peripheral vascular disease, and kidney/liver impairment. Do not stop metoprolol suddenly (risk of rebound angina/arrhythmia); taper only under medical advice. Monitor BP and pulse; seek care for fainting, very slow pulse, breathing difficulty, or swelling.

Other antihypertensives/diuretics (additive hypotension), verapamil/diltiazem/amiodarone/digoxin (increased risk of bradycardia/heart block), clonidine (rebound hypertension on withdrawal), NSAIDs (may reduce antihypertensive effect), CYP2D6 inhibitors like fluoxetine/paroxetine/quinidine (increase metoprolol levels), alcohol (may increase dizziness)

Store below 25°C in a cool, dry place away from light and moisture. Keep in original blister pack and out of reach of children.
