Type 2 diabetes mellitus (adult) to improve blood sugar control when diet and exercise alone or fewer medicines are not adequate
Glimepiride (a sulfonylurea) increases insulin release from the pancreas. Metformin reduces glucose production in the liver and improves insulin sensitivity. Pioglitazone improves insulin sensitivity in muscle and fat by activating PPAR-γ, helping the body use insulin better.
Take exactly as prescribed by your doctor. Usually taken once daily with/after food (preferably with breakfast) to reduce stomach upset; swallow tablet whole (do not crush/chew, especially if sustained/extended-release). Monitor blood glucose regularly.
Hypoglycaemia, nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, headache, dizziness, weight gain, fluid retention/edema
May cause low blood sugar—risk increases with missed meals, alcohol, excess exercise, or with other antidiabetics; keep glucose/sugar source available. Metformin can rarely cause lactic acidosis—risk higher in kidney disease, dehydration, severe infection, heavy alcohol use; dose adjustment/avoid in renal impairment. Pioglitazone may cause fluid retention and can worsen heart failure; avoid/use with caution in heart failure and monitor for swelling/shortness of breath. Periodic monitoring of blood sugar, kidney function, liver function and weight is advised. Inform doctor before surgery or contrast dye procedures (metformin may need temporary stoppage).
Alcohol (increases risk of hypoglycaemia and lactic acidosis), insulin/other antidiabetics (increased hypoglycaemia), diuretics and steroids (may raise blood sugar), rifampicin (may reduce pioglitazone effect), gemfibrozil (may increase pioglitazone levels), iodinated contrast media (risk of metformin-associated lactic acidosis), beta-blockers (may mask hypoglycaemia symptoms)
