Monograph
Telmisartan 40 mg + Chlorthalidone 6.25 mg
Treatment of hypertension (high blood pressure), especially when single-drug therapy is inadequate; helps reduce risk of cardiovascular events associated with uncontrolled BP (as per physician assessment).
Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking angiotensin II action, lowering blood pressure. Chlorthalidone is a thiazide-like diuretic that increases salt and water excretion, reducing blood volume and BP. Together they provide additive blood pressure control.
Take orally exactly as prescribed, usually once daily at the same time each day; may be taken with or without food. Swallow tablet with water; do not stop abruptly without medical advice. If it causes increased urination, taking it in the morning may be preferred.
Dizziness, headache, fatigue, low blood pressure, increased urination, nausea, electrolyte imbalance (low potassium/low sodium), increased uric acid, increased blood glucose, muscle cramps
Avoid in pregnancy; can harm the unborn baby. Monitor kidney function and electrolytes (especially potassium, sodium) and blood pressure regularly. Use caution in dehydration/volume depletion, severe kidney disease, liver impairment, heart failure, diabetes, gout or high uric acid. May cause dizziness—avoid driving if affected. Risk of electrolyte disturbances and worsening renal function, especially with NSAIDs or dehydration. Inform doctor of any swelling of face/lips (angioedema) though rare with ARBs.
NSAIDs (e.g., ibuprofen, diclofenac) may reduce effect and increase kidney risk, other antihypertensives may add to low BP, lithium (increased lithium levels/toxicity risk), potassium supplements/salt substitutes or potassium-sparing diuretics (risk of high potassium due to telmisartan), corticosteroids or amphotericin B (electrolyte imbalance), antidiabetic medicines (may need dose adjustment), alcohol (increased dizziness/low BP)
Store at room temperature (below 25°C), in a dry place away from moisture and direct light. Keep in original blister pack and out of reach of children.
