Glimepiride 2 mg + Metformin Hydrochloride 500 mg (as sustained/modified release) + Pioglitazone 15 mg
Type 2 diabetes mellitus (when diet and exercise alone or single/dual therapy is inadequate), to improve glycaemic control
Glimepiride (sulfonylurea) increases insulin release from pancreatic beta cells. Metformin reduces hepatic glucose production and improves peripheral insulin sensitivity. Pioglitazone (PPAR-γ agonist) improves insulin sensitivity in muscle and adipose tissue, helping reduce insulin resistance.
Oral tablet. Take exactly as prescribed, usually once daily with/after the first main meal to reduce stomach upset and risk of low sugar. Swallow whole (do not crush/chew if modified-release). Monitor blood glucose regularly and follow diet/exercise advice.
Hypoglycaemia, nausea, diarrhoea, abdominal discomfort, headache, dizziness, weight gain, oedema (swelling), upper respiratory tract infection
Risk of hypoglycaemia (higher with missed meals, alcohol, strenuous exercise, elderly, renal/hepatic impairment). Metformin: rare risk of lactic acidosis—avoid in severe kidney impairment, dehydration, severe infection, hypoxia; temporarily stop before/after iodinated contrast or major surgery as advised. Pioglitazone: can cause fluid retention/heart failure worsening—avoid in NYHA class III/IV heart failure; monitor for oedema and weight gain. Long-term metformin may reduce vitamin B12—consider periodic monitoring. Use caution in liver disease; monitor liver enzymes. Not for type 1 diabetes or diabetic ketoacidosis.
Alcohol (increases hypoglycaemia and lactic acidosis risk), insulin/other antidiabetics (additive hypoglycaemia), corticosteroids and thiazide/loop diuretics (may raise glucose), beta-blockers (may mask hypoglycaemia symptoms), ACE inhibitors (may enhance glucose-lowering), rifampicin (reduces pioglitazone levels), gemfibrozil (increases pioglitazone levels), iodinated contrast media (metformin—lactic acidosis risk), cimetidine (may increase metformin levels)
Store below 25°C in a dry place, protected from light and moisture. Keep in original blister pack and out of reach of children.
