Type 2 diabetes mellitus (adult) to control high blood sugar when diet and exercise alone are not sufficient
Glimepiride (a sulfonylurea) increases insulin release from the pancreas and improves peripheral glucose utilization. Metformin (a biguanide) reduces glucose production in the liver and improves insulin sensitivity, lowering blood glucose without usually causing hypoglycaemia on its own.
Take by mouth as advised by doctor, usually once daily with or just after the first main meal to reduce stomach upset and risk of low sugar. Swallow tablet whole with water; do not crush/chew unless advised. Follow diet, exercise, and regular blood glucose monitoring.

Risk of hypoglycaemia (more with missed meals, alcohol, strenuous exercise, elderly, kidney/liver impairment); metformin may rarely cause lactic acidosis—seek urgent care for severe weakness, rapid breathing, persistent vomiting, or abdominal pain. Use caution/avoid in significant renal impairment, severe liver disease, dehydration, severe infection, shock, or hypoxia; temporarily stop metformin before/after iodinated contrast or major surgery as advised. Monitor kidney function, HbA1c, and vitamin B12 (long-term metformin).
Alcohol (increases hypoglycaemia and lactic acidosis risk), insulin/other antidiabetics (additive hypoglycaemia), sulfonamide antibiotics, fluoroquinolones, beta-blockers (mask hypoglycaemia), corticosteroids and thiazide diuretics (raise glucose), ACE inhibitors (may increase hypoglycaemia), rifampicin (reduces effect), cimetidine and other OCT2/MATE inhibitors (increase metformin levels), iodinated contrast media
Store below 25°C in a cool, dry place away from light and moisture. Keep in original blister pack and out of reach of children.
