Treatment of rheumatoid arthritis, psoriatic arthritis, ulcerative colitis (in appropriate patients as prescribed); sometimes used for other autoimmune/inflammatory conditions on specialist advice
Tofacitinib is a Janus kinase (JAK) inhibitor. It blocks JAK-mediated signaling pathways involved in inflammation, thereby reducing immune overactivity and symptoms such as pain, swelling and bowel inflammation.
Tablet for oral use. Take exactly as prescribed by the doctor, at the same time each day, with or without food. Do not crush/chew unless advised; do not stop abruptly without medical advice. Regular blood tests may be required.
Upper respiratory tract infection, headache, diarrhea, nausea, increased cholesterol, elevated liver enzymes, herpes zoster (shingles), hypertension
Prescription-only immunosuppressant. Increases risk of serious infections (TB, fungal, viral), shingles; screen for TB/hepatitis before starting. May increase risk of blood clots (DVT/PE), major cardiovascular events, and certain cancers; use lowest effective dose and avoid/use extreme caution in high-risk patients (history of clots, smokers, cardiovascular disease). Monitor CBC, LFT, lipids; risk of low blood counts. Avoid live vaccines during therapy. Use caution in liver/kidney impairment; dose adjustments may be needed. Seek urgent care for fever, persistent cough, chest pain, shortness of breath, leg swelling, or unusual bleeding/bruising.
Strong CYP3A4 inhibitors (ketoconazole, clarithromycin) may increase levels; strong CYP3A4 inducers (rifampicin, carbamazepine, phenytoin, St John’s wort) may reduce effect; immunosuppressants/biologics (azathioprine, cyclosporine, tacrolimus, TNF inhibitors) increase infection risk and are generally avoided; live vaccines; caution with other drugs increasing clot risk
