Type 2 diabetes mellitus with co-existing hypertension/edema; used to control blood sugar and help lower blood pressure as prescribed.
Metformin reduces glucose production in the liver and improves insulin sensitivity. Glimepiride increases insulin release from the pancreas. Hydrochlorothiazide is a thiazide diuretic that reduces fluid volume and lowers blood pressure by increasing urine output.
Take by mouth as prescribed, usually once daily with food (preferably with/after breakfast) to reduce stomach upset. Swallow whole with water; take at the same time each day. Do not skip meals; monitor blood glucose regularly.
Nausea, vomiting, diarrhea, stomach pain, loss of appetite, dizziness, headache, increased urination, dehydration, low blood sugar (sweating, tremor, hunger), electrolyte imbalance (low potassium), rash
Risk of hypoglycaemia (higher with missed meals, excess exercise, alcohol, or other antidiabetics); monitor glucose. Metformin may rarely cause lactic acidosis—risk increases with kidney disease, dehydration, severe infection, heart/respiratory failure, or alcohol; avoid in severe renal impairment and stop temporarily during acute illness/dehydration. Hydrochlorothiazide may cause low potassium/sodium and increase uric acid (gout) and photosensitivity; monitor electrolytes and kidney function. Use caution in liver disease and in elderly; report severe weakness, muscle cramps, breathing difficulty, or persistent vomiting/diarrhea. Before iodinated contrast scans or major surgery, metformin may need to be withheld per doctor advice.
Alcohol (increases hypoglycaemia/lactic acidosis risk), insulin/other antidiabetics (hypoglycaemia), corticosteroids and beta-agonists (raise glucose), thiazide/loop diuretics (electrolyte changes, may worsen glycaemia), ACE inhibitors/ARBs (may enhance glucose-lowering, affect potassium), NSAIDs (renal risk), iodinated contrast media (lactic acidosis risk with metformin), lithium (increased lithium levels with thiazides), digoxin (electrolyte-related toxicity), sulfonamide allergy cross-reactivity (HCTZ)
