Important Warnings

Risk of hypoglycaemia (higher with missed meals, alcohol, strenuous exercise, elderly, renal/hepatic impairment). Metformin may rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, muscle pain, abdominal pain, or unusual sleepiness. Avoid in severe kidney disease; dose adjustment/monitoring required in renal impairment. Use caution in liver disease, heart failure, dehydration, and during severe infection or surgery. Temporarily stop metformin before/after iodinated contrast procedures as advised by doctor. Not for type 1 diabetes or diabetic ketoacidosis.

Alcohol (increases risk of hypoglycaemia/lactic acidosis)
iodinated contrast media
insulin/other antidiabetics (additive hypoglycaemia)
beta-blockers (mask hypoglycaemia symptoms)
ACE inhibitors (may enhance glucose-lowering)
diuretics and corticosteroids (may raise glucose)
fluoroquinolones (glucose disturbances)
azole antifungals/clarithromycin (may increase sulfonylurea effect)
rifampicin (may reduce effect)
NSAIDs and salicylates (may increase hypoglycaemia)
cimetidine (may increase metformin levels)

Always tell your doctor about all medicines you are taking, including over-the-counter drugs and supplements.
