Telmisartan 40 mg + Hydrochlorothiazide 12.5 mg
Treatment of essential hypertension (high blood pressure), especially when blood pressure is not adequately controlled with telmisartan alone; helps reduce risk of cardiovascular complications related to uncontrolled hypertension.
Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking angiotensin II, lowering blood pressure. Hydrochlorothiazide is a thiazide diuretic that increases salt and water excretion via kidneys, reducing fluid volume and blood pressure. The combination provides additive BP-lowering effect.
Take as prescribed by a doctor, usually once daily at the same time each day. Can be taken with or without food; swallow tablet with water. Because it contains a diuretic, taking it in the morning is often preferred to reduce nighttime urination; do not stop suddenly without medical advice.

Not recommended in pregnancy (can harm fetus); avoid if allergic to telmisartan, hydrochlorothiazide or sulfonamides. Use caution in kidney or liver disease, dehydration, low blood pressure, heart failure, renal artery stenosis, diabetes, gout, and in elderly. Monitor blood pressure, kidney function and electrolytes periodically; risk of dehydration and electrolyte disturbances—drink adequate fluids unless advised otherwise. May cause dizziness—avoid driving/operating machinery until effects are known. Hydrochlorothiazide may increase blood sugar and uric acid; telmisartan may increase potassium (risk higher with kidney disease or potassium-sparing drugs).
