Neuropathic (nerve) pain such as diabetic peripheral neuropathy, post-herpetic neuralgia, radiculopathy/sciatica-related neuropathic pain, and other chronic neuropathic pain conditions as prescribed.
Pregabalin reduces release of excitatory neurotransmitters by binding to the alpha-2-delta subunit of voltage-gated calcium channels, decreasing abnormal nerve signaling. Nortriptyline (a tricyclic antidepressant) increases norepinephrine and serotonin in the spinal cord, enhancing descending pain inhibition. The combination provides additive relief in neuropathic pain.
Take orally as prescribed, usually once daily at bedtime or as directed. Can be taken with or without food; swallow whole with water. Do not stop suddenly—taper only on medical advice.
Dizziness, sleepiness (somnolence), dry mouth, constipation, blurred vision, weight gain, swelling of legs/feet (peripheral edema), fatigue, difficulty concentrating
May cause dizziness/somnolence—avoid driving or operating machinery until you know its effects. Do not stop abruptly (risk of withdrawal symptoms/seizures); taper gradually. Use with caution in kidney disease (pregabalin dose adjustment), glaucoma/urinary retention/prostatic enlargement (nortriptyline anticholinergic effects), heart disease/arrhythmias, and in the elderly (falls, confusion). Monitor mood—seek help for worsening depression or suicidal thoughts. Risk of dependence/misuse with pregabalin; use strictly as prescribed.
Alcohol and other CNS depressants (opioids, benzodiazepines, sedating antihistamines) increase drowsiness/respiratory depression risk; MAO inhibitors (contraindicated/avoid; risk of serious reactions); other antidepressants/serotonergic drugs (risk of serotonin syndrome), anticholinergic drugs (additive dry mouth/constipation/urinary retention), QT-prolonging medicines/antiarrhythmics (arrhythmia risk), gabapentin or other sedatives (additive CNS effects).
Store at room temperature (below 25°C), protected from light and moisture. Keep in original blister pack and out of reach of children.
