Type 2 diabetes mellitus in adults when diet and exercise alone or monotherapy is insufficient; helps control blood glucose levels.
Glimepiride (a sulfonylurea) increases insulin release from the pancreas and improves peripheral glucose utilization. Metformin reduces glucose production by the liver and improves insulin sensitivity, lowering blood sugar without usually causing hypoglycaemia when used alone.
Oral tablet. Take exactly as prescribed, usually once daily with or immediately after food (preferably breakfast) to reduce stomach upset. Swallow whole if SR/ER; do not crush or chew. Monitor blood glucose regularly.

Risk of hypoglycaemia (higher with missed meals, excess exercise, alcohol, or other antidiabetics); avoid skipping meals. Metformin may rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, muscle pain, unusual sleepiness, or stomach pain; risk increases with kidney or liver impairment, dehydration, severe infection, or heavy alcohol use. Use with caution in elderly and in renal impairment; kidney function should be assessed before and during treatment. Withhold metformin temporarily before/after iodinated contrast imaging and major surgery as advised by doctor. Not for type 1 diabetes or diabetic ketoacidosis.
Insulin/other antidiabetics (increased hypoglycaemia), alcohol (increased hypoglycaemia/lactic acidosis risk), corticosteroids and thiazide diuretics (raise glucose), beta-blockers (mask hypoglycaemia symptoms), ACE inhibitors (may increase hypoglycaemia), fluconazole and other CYP2C9 inhibitors (increase glimepiride effect), rifampicin (reduce glimepiride effect), iodinated contrast media (lactic acidosis risk with metformin), cimetidine (increase metformin levels)
