Treatment of hypertension (high blood pressure); helps reduce risk of cardiovascular complications such as stroke and heart attack associated with uncontrolled BP
Cilnidipine is a calcium channel blocker that inhibits L-type and N-type calcium channels in vascular smooth muscle. This relaxes and widens blood vessels, lowering peripheral resistance and blood pressure; N-type blockade may also reduce sympathetic nerve activity.
Take by mouth as prescribed, usually once daily at the same time each day. Can be taken with or without food; swallow tablet whole with water. Do not stop abruptly without medical advice; monitor blood pressure regularly.
Headache, dizziness, flushing, ankle/foot swelling (peripheral edema), fatigue, palpitations, nausea
Use with caution in liver impairment; dose adjustment/monitoring may be needed. May cause dizziness—avoid driving or operating machinery until you know the effect. Monitor for excessive fall in blood pressure, especially at initiation or with other antihypertensives. Use cautiously in severe aortic stenosis, heart failure, or if prone to hypotension. Do not discontinue suddenly without doctor advice.
Other antihypertensives (additive hypotension), strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin, ritonavir) may increase cilnidipine levels, CYP3A4 inducers (e.g., rifampicin, carbamazepine, phenytoin) may reduce effect, grapefruit juice may increase levels, NSAIDs may reduce antihypertensive effect in some patients
