What is the HLA B27 Test?

The HLA-B27 test checks whether your white blood cells carry the HLA-B27 marker, a genetic marker linked to ankylosing spondylitis and related inflammatory conditions. The result is positive or negative, and many healthy people are positive, so it only supports a diagnosis and never makes one.

Positive
The marker is present. A minority of carriers ever develop disease, so a positive result with back pain only adds weight to the diagnosis. Imaging and a rheumatologist's examination decide it.
Negative
Makes ankylosing spondylitis less likely, but does not rule out spondyloarthritis, as a considerable share of patients are HLA-B27 negative.
