When the rules ask for Form 1A

Form 1 is the driver's own declaration. Form 1A is signed by a doctor. Form 1 and Form 1A compared.
A medical certificate in Form 1A, completed by a registered medical practitioner, goes with an application for a licence to drive a transport vehicle, and with applications from people aged 40 or more.
Central Motor Vehicles Rules, 1989, Rule 5, in plain words

In practice the driver's Sarathi (Parivahan) application shows whether Form 1A is needed, at the stage where it asks for the medical certificate. For a commercial fleet that means most drivers at most renewals.

The doctor completes and signs the form. The driver then uploads it to their own application, or hands it in where their RTO asks for paper.

No fixed validity. We do not set a validity period for Form 1A, and our register shows only the date it was issued. The certificate carries the date of the examination; the RTO decides how recent it must be. Ask drivers to submit it soon after it is issued.

Acceptance. The RTO or licensing authority decides whether to accept a certificate. Nobody can promise it in advance, and we do not.

What the doctor checks

The questions are the ones printed on Form 1-A. The doctor answers each one for the driver.

On the form

  • Any defect of vision, and whether glasses correct it
  • Can the driver read a car number plate at 25 metres in good daylight
  • Deafness that would stop them hearing ordinary sound signals
  • Night blindness
  • Any defect, deformity or loss of a limb that would affect driving
  • Colour vision: no severe or total colour blindness
  • Blood group and Rh factor, only if the driver wants them on the licence

What each driver needs ready

  • The Sarathi (Parivahan) application number
  • The state of their RTO
  • Two identification marks, such as a mole or a scar
  • Their glasses or hearing aid, if they use one
  • A phone, and the code you sent them

The doctor may ask for an in-person or specialist check before signing. Form 1-A itself asks for reaction time, side vision and glare recovery to be examined in person for drivers of vehicles carrying dangerous or hazardous goods, so tanker and hazardous-goods drivers should expect a clinic visit.

How it works

  1. Step 1

    Order the codes

Choose how many drivers and pay once by UPI, card or net banking. Your codes appear on a private order page as soon as the payment lands.
  2. Step 2

    Send each driver a code

Each code on the order page has a WhatsApp button and a copy button. Note the driver's name and mobile against the code so you know who has which.
  3. Step 3

The driver applies from their phone

They open the link, fill in their Form 1A application and use the code instead of paying. A registered medical practitioner assesses them. Drivers can also enter a code at pingmedoc.com/corporate/redeem.
  4. Step 4

    See who is done

The order page shows every code: not used yet, used, or certificate issued, with the issue date. Download it as a CSV for your records.

You see which codes are unused, which have been used and which have ended in a certificate, with the date it was issued. The list downloads as a CSV with each driver's name, mobile and status, ready for your own records or an audit.

You do not see medical details. You learn that a code was used and whether a certificate was issued, never why an application did not end in one.

Form 1A shows the issue date only, with no expiry date, so there are no renewal reminders for Form 1A. The 30-day and 7-day reminders on our year-long certificates, such as the food handler certificate, do not apply here.

Anyone can check a certificate

Every Form 1A we issue carries a QR code and a certificate number. An RTO officer, a fleet aggregator, an insurer or your own team can scan it, or enter the number at pingmedoc.com/medical-certificates/verify, and see whether it is genuine.

A forged medical certificate is an offence under the forgery provisions of the Bharatiya Nyaya Sanhita and can get a licence application rejected or a licence cancelled. The QR lets anyone tell a real one from a copy.

What we can't promise

Better to know now than at the RTO counter.
  • Acceptance. The RTO or licensing authority decides whether to accept a certificate. We cannot guarantee it.
  • States. We issue Form 1A for licences in Tamil Nadu, Andhra Pradesh, Karnataka, Kerala, Gujarat, Delhi, Madhya Pradesh, Uttar Pradesh, West Bengal, Haryana, Punjab, Telangana, Rajasthan, Odisha and Chhattisgarh today. A driver licensed elsewhere cannot use a code yet; tell us in the form below and we will tell you when their state opens.
  • A certificate for every driver. The doctor signs only what the medical facts support, and may ask for an in-person or specialist check first.
  • Anything at the RTO itself. We do not book RTO slots, arrange driving tests or pay licence fees.
  • Faster packages. A code covers the Standard package only.

Questions fleet owners ask

Which drivers need a Form 1A?
Under the Central Motor Vehicles Rules, 1989, Form 1A is the medical certificate a registered medical practitioner signs. The Sarathi (Parivahan) application asks for it from drivers applying for, renewing or adding a transport (commercial) licence, and from drivers aged 40 or more. Each driver's own application shows whether it is needed.
Is a Form 1A from an online consultation accepted by the RTO?
Form 1A must be completed by a registered medical practitioner, and the Telemedicine Practice Guidelines, 2020 let registered doctors consult by video. Whether a certificate is accepted is decided by the RTO or licensing authority, not by us, and we cannot guarantee it. Some RTOs or cases need an in-person examination; the doctor tells the driver if that applies.
How long is a Form 1A valid?
We do not set a validity period for Form 1A, and our register shows its issue date only. The certificate carries the date of the examination, and the RTO decides how recent it must be. Ask each driver to submit it soon after it is issued.
What does it cost per driver?
₹999 per driver for 1 to 4 drivers, the same as one driver pays on our website. The price per driver drops from 5 drivers and is ₹749 from 100. No GST is added. Online orders go up to 500 drivers; above that we quote.
Is there a minimum order?
No. You can order a code for a single driver. Below 5 drivers you pay the website price and still get one payment, one invoice and the order page that tracks every code.
Do drivers have to visit a clinic?
No. Each driver applies from their own phone and a registered medical practitioner assesses them online. The doctor can ask for an in-person or specialist check before signing, for example when a vision or hearing problem needs a proper test.
What does each driver need to have ready?
Their Sarathi (Parivahan) application number, the state of their RTO, two identification marks such as a mole or a scar, and their glasses or hearing aid if they use one. They must be 18 or older.
What if a driver is found unfit?
The doctor will not sign a certificate the medical facts do not support. The code counts as used, because it paid for the doctor's assessment. On your order page you see that the code was used and no certificate was issued; you are not told the medical reason.
Which states do you issue Form 1A for?
Today we issue Form 1A for licences in Tamil Nadu, Andhra Pradesh, Karnataka, Kerala, Gujarat, Delhi, Madhya Pradesh, Uttar Pradesh, West Bengal, Haryana, Punjab, Telangana, Rajasthan, Odisha and Chhattisgarh. The list changes as states open, and this page shows the current one.
Do we get an invoice for the company?
Yes. Every order has an invoice from PingMeDoc HealthTech Private Limited on its order page, with your company name and GSTIN if you give one. No GST is charged on Form 1A, so it is issued as a bill of supply rather than a tax invoice.
Can one code be used twice, or by the wrong driver?
Each code pays for one application and then shows as used, with the date. Anyone who has a code can use it, so send each code to one driver and note on the order page who has which.
Can drivers from different states be in one order?
Yes. A code is not tied to a state. Each driver applies for their own RTO, as long as we issue Form 1A for that state.
