Monograph
Human insulin (rDNA origin) 40 IU/mL (Regular/soluble insulin) injection
Treatment of diabetes mellitus (Type 1 and Type 2) to control high blood glucose; management of diabetic ketoacidosis (under medical supervision); perioperative/acute illness hyperglycaemia as directed by a doctor
Regular human insulin replaces/supplements insulin in the body. It helps glucose enter muscle and fat cells and reduces glucose production by the liver, thereby lowering blood sugar levels. It is a short-acting insulin with relatively quick onset and shorter duration compared with intermediate/long-acting insulins.
Subcutaneous injection as prescribed, usually 30 minutes before meals. Dose is individualized based on blood glucose, diet and activity; rotate injection sites (abdomen, thigh, upper arm). May be used IV only in hospital settings by trained professionals; use only insulin syringes calibrated for U-40 insulin.
Hypoglycaemia, injection site pain/redness/swelling, lipodystrophy (lumps/skin thickening), weight gain, rash/itching, peripheral oedema
Hypoglycaemia is the most important risk—carry glucose/sugar and monitor blood glucose regularly. Use correct strength (U-40) and matching syringe to avoid dosing errors; do not share needles/syringes. Caution in renal or hepatic impairment, elderly, irregular meals, increased exercise, and in patients with autonomic neuropathy (reduced hypoglycaemia warning signs). Seek urgent care for severe allergy, recurrent severe hypoglycaemia, or hyperglycaemia/ketosis symptoms; do not stop insulin abruptly.
Other antidiabetics (additive hypoglycaemia), beta-blockers (may mask hypoglycaemia), ACE inhibitors/ARBs (may increase hypoglycaemia risk), corticosteroids/thyroid hormones/oral contraceptives/thiazide diuretics (may increase blood glucose), MAO inhibitors/salicylates (may enhance insulin effect), alcohol (can cause hypoglycaemia), sympathomimetics (may raise glucose)
Unopened vials: store refrigerated at 2–8°C; do not freeze. Protect from light. After opening/in-use: keep as per label (often refrigerated or at room temperature below 25–30°C) and discard after the in-use period; keep vial tightly closed and avoid heat.
Actrapid 40 IU/mL Injection (Novo Nordisk), Huminsulin R 40 IU/mL Injection (Eli Lilly), Insugen-R 40 IU/mL Injection (Biocon), Wosulin-R 40 IU/mL Injection (Wockhardt)
