Type 2 diabetes mellitus to improve blood glucose control in adults when diet and exercise alone or monotherapy is insufficient.
Vildagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), enhancing glucose-dependent insulin release and reducing glucagon secretion. Metformin decreases hepatic glucose production and improves peripheral insulin sensitivity, lowering blood glucose without usually causing hypoglycaemia when used alone.
Oral tablet. Take exactly as prescribed, usually with meals to reduce stomach upset. Swallow whole with water; do not crush/chew. Dose is individualized based on blood sugar control and kidney function.
Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, metallic taste, headache, dizziness, nasopharyngitis, hypoglycaemia (especially with other antidiabetics), increased liver enzymes
Contraindicated in severe renal impairment; assess eGFR before start and periodically. Risk of lactic acidosis with metformin (more likely in kidney/liver disease, dehydration, sepsis, hypoxia, excessive alcohol); stop and seek care if deep/rapid breathing, severe weakness, abdominal pain occur. Monitor liver function; vildagliptin may rarely cause hepatitis—discontinue if jaundice or persistent LFT elevation. Temporarily stop metformin before/at time of iodinated contrast procedures or major surgery and restart after renal function is stable. Use caution in elderly and in conditions causing dehydration; monitor for pancreatitis symptoms (severe persistent abdominal pain).
Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate/acetazolamide), cimetidine, diuretics, corticosteroids, thyroid hormones, beta-agonists, other antidiabetics (insulin/sulfonylureas increase hypoglycaemia risk), ACE inhibitors (may enhance glucose-lowering), drugs affecting renal function (NSAIDs/ACEi/ARB)
