Treatment of acute promyelocytic leukemia (APL, M3 subtype) in patients with t(15;17) / PML-RARA, typically for induction of remission under specialist supervision

Tretinoin is a retinoid that binds to retinoic acid receptors and promotes differentiation and maturation of malignant promyelocytes. In APL it helps overcome the differentiation block caused by the PML-RARA fusion, reducing abnormal blasts and helping restore normal blood cell development.

Oral capsule. Dose and duration are strictly as prescribed by an oncologist (commonly weight-based in APL). Swallow whole with water, preferably with food to improve absorption; take at the same times each day. Do not crush or open the capsule.

Headache, fever, skin rash, dry skin/lips, nausea, vomiting, diarrhea, abdominal pain, bone/joint pain, fatigue, peripheral edema, elevated liver enzymes, increased blood lipids

For use only under specialist (oncology/hematology) supervision. Risk of ATRA (differentiation) syndrome/retinoic acid syndrome (fever, breathing difficulty, weight gain, edema, low blood pressure) which can be life-threatening—seek urgent care. Highly teratogenic: strict pregnancy prevention required; avoid in pregnancy and do not breastfeed. Monitor CBC, liver function, lipids, and for blood clots; caution in hepatic impairment, hyperlipidemia, and pancreatitis risk. May cause dizziness/visual disturbances—use caution while driving. Avoid vitamin A supplements to reduce toxicity.

Tetracyclines (risk of increased intracranial pressure), vitamin A/other retinoids (additive toxicity), azole antifungals (e.g., ketoconazole/itraconazole) and macrolides (may increase levels), CYP inducers (e.g., rifampicin, phenytoin, carbamazepine, phenobarbital) may reduce effect, drugs increasing thrombotic risk (e.g., hormonal contraceptives)

Store below 25°C, protected from light and moisture. Keep in original blister pack. Keep out of reach of children.
