Treatment of severe psoriasis (especially pustular psoriasis), disorders of keratinization such as ichthyosis and severe palmoplantar keratoderma, when other treatments are unsuitable

Acitretin is an oral retinoid (vitamin A derivative) that normalizes epidermal cell growth and differentiation. It reduces excessive keratinization and inflammation, improving scaling, thickness, and redness of psoriatic lesions.

Take by mouth exactly as prescribed, preferably with meals (fat-containing food improves absorption). Swallow whole with water; take at the same time each day. Dose and duration are individualized; do not stop or change dose without medical advice.

Dry lips, dry skin, peeling, itching, hair thinning, dry eyes, nosebleeds, headache, increased liver enzymes, increased triglycerides/cholesterol, photosensitivity

High teratogenic risk: contraindicated in pregnancy; effective contraception required during treatment and for a prolonged period after stopping (as advised by doctor). Do not donate blood during therapy and for the advised period after stopping. Avoid vitamin A supplements and other retinoids (risk of toxicity). Monitor liver function tests and lipid profile; use caution in liver disease, high cholesterol/triglycerides, diabetes, depression, or alcohol use. May cause reduced night vision; caution while driving. Use sunscreen and avoid excessive sunlight.

Alcohol (increases formation of etretinate and prolongs teratogenic risk), tetracyclines (risk of intracranial hypertension), methotrexate (hepatotoxicity risk), vitamin A/other retinoids (additive toxicity), phenytoin (increased toxicity risk), corticosteroids (may affect bone/lipid metabolism), lipid-lowering medicines (monitor lipids)

Store at room temperature (below 25°C), protected from light and moisture. Keep in original pack and out of reach of children.
