Treatment of severe psoriasis (including erythrodermic and pustular psoriasis) and other severe disorders of keratinization (e.g., ichthyosis, palmoplantar keratoderma) when other therapies are inadequate.
Acitretin is an oral retinoid that normalizes epidermal cell growth and differentiation and reduces abnormal keratinization. It also has anti-inflammatory effects that help improve psoriatic plaques and scaling.
Take orally exactly as prescribed, usually once daily with food (preferably a fatty meal) to improve absorption. Swallow capsule whole; do not crush or chew. Strictly follow contraception and monitoring instructions.
Dry lips (cheilitis),dry skin,peeling,itching,photosensitivity,hair thinning,eye dryness/conjunctivitis,nose dryness/epistaxis,headache,elevated liver enzymes,increased triglycerides and cholesterol,muscle/joint pain
Severe teratogenic risk: contraindicated in pregnancy; effective contraception required during therapy and for an extended period after stopping (per prescriber guidance). Do not donate blood during treatment and for a prolonged period after discontinuation. Avoid alcohol during treatment and for at least 2 months after stopping (can form etretinate, prolonging teratogenic risk). Use with caution in liver disease, hyperlipidemia, diabetes, obesity, depression; monitor LFTs and lipid profile regularly. May cause night vision impairment; caution while driving. Avoid excess sun/UV exposure. Do not use with other retinoids or vitamin A due to toxicity.
Alcohol (increases conversion to etretinate),tetracyclines (risk of intracranial hypertension),vitamin A/other retinoids (additive toxicity),methotrexate (increased hepatotoxicity),phenytoin (protein-binding interaction),progestin-only mini-pill may be less reliable (use combined/dual contraception),lipid-lowering agents and hepatotoxic drugs (additive risk)
