Gastroesophageal reflux disease (GERD), acid reflux/heartburn, erosive esophagitis, peptic ulcer disease (gastric/duodenal ulcer), Zollinger–Ellison syndrome and other hypersecretory conditions
Pantoprazole is a proton pump inhibitor (PPI). It irreversibly blocks the H+/K+ ATPase (proton pump) in gastric parietal cells, reducing stomach acid production and allowing ulcers/erosions to heal.
Tablet: take as prescribed, usually 40 mg once daily 30–60 minutes before food (preferably before breakfast). Swallow whole; do not crush or chew (enteric-coated). Duration depends on condition; complete the course.
Headache, diarrhea, nausea, abdominal pain, flatulence, vomiting, dizziness
Use with caution in severe liver disease. Long-term use may increase risk of low magnesium, vitamin B12 deficiency, bone fractures, and Clostridioides difficile–associated diarrhea; consult doctor if watery diarrhea persists. Seek medical advice for alarm symptoms (unintentional weight loss, persistent vomiting, swallowing difficulty, blood in vomit/black stools). May rarely cause acute interstitial nephritis or severe skin reactions. Do not use for immediate relief of acute heartburn.
Atazanavir/nelfinavir (reduced effect; avoid), warfarin (monitor INR), methotrexate (high-dose; may increase levels), digoxin/diuretics (risk of hypomagnesemia), ketoconazole/itraconazole/posaconazole and erlotinib (reduced absorption), clopidogrel (generally less interaction than omeprazole but caution), iron salts (reduced absorption)
