Treatment of acid-related disorders when oral therapy is not possible: gastroesophageal reflux disease (GERD), erosive esophagitis, peptic/duodenal ulcer, Zollinger–Ellison syndrome; prevention/treatment of stress-related mucosal bleeding in critically ill patients (as per clinician).
Pantoprazole is a proton pump inhibitor (PPI). It irreversibly blocks the gastric H+/K+ ATPase (proton pump) in parietal cells, reducing basal and stimulated acid secretion and allowing ulcers/erosions to heal.
Parenteral use only (IV) by a healthcare professional. Typically given as slow IV injection or IV infusion once daily; switch to oral pantoprazole as soon as feasible. Can be given irrespective of meals.
Headache, diarrhea, nausea, abdominal pain, vomiting, dizziness, injection site pain/phlebitis, flatulence
Use with caution in severe liver impairment. Long-term/high-dose PPI use may be associated with low magnesium, vitamin B12 deficiency, increased fracture risk, C. difficile-associated diarrhea, and kidney injury (interstitial nephritis). May mask symptoms of gastric malignancy—seek evaluation if alarm symptoms (weight loss, persistent vomiting, GI bleeding). Hypersensitivity reactions are possible. IV administration should be supervised; monitor for local infusion reactions.
May reduce absorption of drugs needing acidic pH (e.g., ketoconazole, itraconazole, posaconazole, erlotinib). Avoid with atazanavir/nelfinavir (reduced antiviral levels). High-dose methotrexate levels may increase—temporary PPI interruption may be needed. Caution with warfarin (INR changes reported).
Store below 25°C, protected from light and moisture. Do not freeze. Reconstituted/diluted solution should be used as per label recommendations; discard if discolored or particulate.
