Type 2 diabetes mellitus: to improve blood glucose control along with diet and exercise, when dual therapy is appropriate
Sitagliptin is a DPP-4 inhibitor that increases incretin hormones, enhancing glucose-dependent insulin release and reducing glucagon secretion. Metformin decreases hepatic glucose production and improves insulin sensitivity, lowering intestinal glucose absorption.
Oral tablet. Usually taken with meals to reduce stomach upset; swallow whole with water. Dose and frequency as prescribed by the doctor; do not crush/chew. Monitor blood glucose regularly.
Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, metallic taste, headache, dizziness, upper respiratory tract infection
Risk of lactic acidosis with metformin (seek urgent care for rapid breathing, severe weakness, muscle pain, unusual sleepiness). Use with caution in kidney or liver impairment; dose adjustment/avoid in low eGFR as advised by physician. Temporarily stop before/after iodinated contrast studies or major surgery as directed. Pancreatitis risk with sitagliptin (severe persistent abdominal pain); severe joint pain and hypersensitivity reactions can occur. Monitor vitamin B12 with long-term metformin use. Avoid in type 1 diabetes or diabetic ketoacidosis.
Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cimetidine, digoxin, insulin or sulfonylureas (increased hypoglycaemia risk), ACE inhibitors/ARBs and diuretics (may affect kidney function)
