Type 2 diabetes mellitus (adult) to improve blood glucose control when diet and exercise alone are insufficient; used when combination therapy with a sulfonylurea and metformin is appropriate.
Gliclazide (a sulfonylurea) stimulates the pancreas to release insulin, helping lower blood glucose. Metformin reduces glucose production in the liver and improves insulin sensitivity, reducing insulin resistance and lowering glucose absorption from the gut.
Take by mouth as advised by the doctor, usually once daily with or after food (preferably with breakfast) to reduce stomach upset. Swallow the XR tablet whole; do not crush, chew, or split. Monitor blood glucose regularly and follow diet/exercise advice.
Nausea, vomiting, diarrhea, abdominal pain, loss of appetite, metallic taste, hypoglycemia (low blood sugar), headache, dizziness, weight gain, rash
Risk of hypoglycemia (especially if meals are skipped, with alcohol, or with other antidiabetics); carry a fast-acting sugar source. Metformin may rarely cause lactic acidosis—seek urgent care for fast breathing, severe weakness, abdominal pain, or unusual sleepiness; higher risk with kidney disease, dehydration, severe infection, heart/liver failure. Use caution/avoid in significant renal impairment; kidney function should be assessed before and during therapy. Stop temporarily before major surgery or iodinated contrast procedures as advised. Use caution in elderly, malnourished, and those with adrenal/pituitary insufficiency. Not for type 1 diabetes or diabetic ketoacidosis.
Alcohol (increases risk of hypoglycemia and lactic acidosis), iodinated contrast media, insulin/other antidiabetics (increased hypoglycemia), corticosteroids (may raise glucose), thiazide/loop diuretics (may raise glucose; dehydration risk), ACE inhibitors (may increase hypoglycemia), beta-blockers (mask hypoglycemia symptoms), fluoroquinolones (glucose disturbances), azole antifungals and macrolides (may increase sulfonylurea effect), cimetidine (may increase metformin levels)
Store below 25°C in a cool, dry place; protect from moisture and light; keep in original blister pack and out of reach of children.
