Type 2 diabetes mellitus (as an adjunct to diet and exercise) to improve glycaemic control
Gliclazide is a sulfonylurea that stimulates insulin release from pancreatic beta cells by closing ATP-sensitive potassium channels. This increases insulin secretion and helps lower blood glucose levels, particularly post‑meal glucose.
Oral tablet. Take exactly as prescribed, usually once daily with breakfast/first main meal; swallow whole with water (do not crush/chew if modified-release). Maintain regular meals and monitor blood glucose; if a dose is missed, take the next dose at the usual time—do not double.
Hypoglycaemia, dizziness, headache, nausea, abdominal pain, diarrhoea, constipation, weight gain, skin rash/itching
Risk of hypoglycaemia especially with missed meals, strenuous exercise, alcohol intake, dose errors, renal/hepatic impairment, or when combined with other antidiabetic drugs. Use with caution in elderly, malnourished patients, and those with kidney or liver disease. Not indicated for type 1 diabetes or diabetic ketoacidosis. Seek medical help for severe hypoglycaemia, allergic reactions, jaundice, or unusual bruising/bleeding. During acute illness/surgery, treatment may need adjustment.
Other antidiabetics (insulin, metformin, DPP-4 inhibitors, GLP-1 agonists) increase hypoglycaemia risk; alcohol increases hypoglycaemia risk; beta-blockers may mask hypoglycaemia symptoms; corticosteroids and thiazide diuretics may raise glucose; fluconazole and some macrolide/quinolone antibiotics can increase hypoglycaemic effect; warfarin effect may be altered—monitor INR
