Type 2 diabetes mellitus: adjunct to diet and exercise to improve glycaemic control when dual therapy with an SGLT2 inhibitor and a DPP-4 inhibitor is appropriate.
Dapagliflozin (SGLT2 inhibitor) lowers blood glucose by increasing urinary glucose excretion via the kidneys. Vildagliptin (DPP-4 inhibitor) increases incretin levels, enhancing glucose-dependent insulin release and reducing glucagon secretion. Together they improve fasting and post‑meal glucose control with a low risk of hypoglycaemia when not used with insulin/sulfonylureas.
Tablet (SR): take exactly as prescribed, usually once daily; swallow whole without crushing/chewing. Can be taken with or without food (preferably at the same time each day). Maintain adequate hydration; monitor blood sugar regularly.
Genital fungal infection, urinary tract infection, increased urination, thirst, dizziness, headache, nausea, diarrhoea, upper respiratory tract infection
Prescription medicine. Not for type 1 diabetes or diabetic ketoacidosis. Risk of dehydration/low blood pressure—use caution in elderly, on diuretics, or with low fluid intake. SGLT2 inhibitors can rarely cause ketoacidosis (even with near-normal sugars); seek urgent care for nausea, vomiting, abdominal pain, rapid breathing. Increased risk of genital infections; maintain hygiene. Monitor kidney function—avoid/consult doctor in severe renal impairment. DPP-4 inhibitors may rarely cause pancreatitis or severe joint pain; stop and seek medical advice if severe abdominal pain. Monitor for hypoglycaemia if used with insulin/sulfonylureas. Before surgery or acute illness, doctor may advise temporary stoppage.
Insulin, sulfonylureas (increased hypoglycaemia risk), diuretics (increased dehydration/hypotension risk), strong CYP3A4/UGT inducers like rifampicin/phenytoin/carbamazepine (may reduce dapagliflozin effect), digoxin (monitor if co-administered), other antidiabetic medicines/alcohol (may increase hypoglycaemia risk)
