Treatment of insomnia in adults, including difficulty falling asleep (sleep-onset insomnia) and/or staying asleep (sleep-maintenance insomnia).

Lemborexant is a dual orexin receptor antagonist (DORA) that blocks orexin-1 and orexin-2 receptors. By inhibiting orexin signaling, it reduces wakefulness and helps initiate and maintain sleep.

Oral tablet. Take once daily immediately before bedtime, only when you can stay in bed for at least 7 hours. Can be taken with or without food, but avoid taking with or soon after a meal as it may delay onset. Swallow whole; do not crush or chew.

May cause next-day drowsiness and impaired driving/operating machinery; risk is higher with 10 mg, with insufficient sleep time, or with other CNS depressants. Contraindicated/avoid in narcolepsy. Use with caution in depression, history of suicidal thoughts, and in patients with severe hepatic impairment or severe sleep apnea/respiratory disorders (may worsen). Complex sleep behaviors (e.g., sleep-walking, sleep-driving) can occur—stop and seek medical advice if they occur. Elderly may be more sensitive to sedation and falls.

CNS depressants (alcohol, opioids, benzodiazepines, antihistamines) increase sedation; strong/moderate CYP3A inhibitors (e.g., ketoconazole, itraconazole, clarithromycin, ritonavir, diltiazem, verapamil) may increase levels—avoid/adjust as advised; CYP3A inducers (e.g., rifampicin, carbamazepine, phenytoin, St John’s wort) may reduce effect—avoid; grapefruit juice may increase levels—avoid or limit.

Store at room temperature (below 25°C), in original pack, protected from moisture and light. Keep out of reach of children.
