Dapagliflozin is an SGLT2 inhibitor that reduces glucose reabsorption in the kidneys, increasing urinary glucose excretion and lowering blood sugar. Metformin decreases hepatic glucose production and improves insulin sensitivity, reducing glucose levels.
Oral tablet. Take exactly as prescribed by the doctor, usually once daily with food (especially if metformin is SR/ER) to reduce stomach upset. Swallow whole; do not crush/chew sustained/extended-release tablets. Maintain adequate hydration.

Not for type 1 diabetes or diabetic ketoacidosis. Risk of lactic acidosis with metformin (higher risk in severe kidney impairment, dehydration, sepsis, heavy alcohol use); stop and seek help for severe weakness, rapid breathing, abdominal pain. SGLT2 inhibitors can cause genital infections, dehydration/low BP, and (rare) ketoacidosis even with normal sugars—seek care for nausea/vomiting, abdominal pain, rapid breathing. Monitor kidney function before and during therapy; not recommended in severe renal impairment. Hold before major surgery or during acute illness/poor intake; avoid dehydration. Use caution in elderly and with diuretics. Hypoglycaemia risk increases if used with insulin or sulfonylureas.
Insulin and sulfonylureas (increased hypoglycaemia risk), diuretics (increased dehydration/hypotension risk), ACE inhibitors/ARBs and NSAIDs (kidney function risk when combined with dehydration), iodinated contrast media (increase lactic acidosis risk—temporary discontinuation of metformin may be needed), alcohol (increases lactic acidosis risk), carbonic anhydrase inhibitors like topiramate (may increase acidosis risk), digoxin (metformin may increase levels—monitor)
