Type 2 diabetes mellitus (to control blood glucose), prediabetes/insulin resistance (as prescribed), polycystic ovary syndrome (PCOS) (off-label, as prescribed)
Metformin decreases glucose production in the liver and improves the body’s sensitivity to insulin, helping cells use glucose better. It also reduces absorption of glucose from the intestine, leading to lower blood sugar levels.
Oral tablet. Take exactly as prescribed, usually with or after meals to reduce stomach upset. Swallow with water; do not crush unless advised. Dose is individualized based on blood sugar and kidney function; regular monitoring is needed.
Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, metallic taste, gas/bloating, headache
Use with caution in kidney disease; periodic renal function monitoring is required. Rare risk of lactic acidosis—seek urgent care for fast/deep breathing, severe weakness, muscle pain, abdominal pain, or unusual sleepiness. Temporarily stop metformin before/after iodinated contrast imaging and major surgery as advised. Use caution in liver disease, dehydration, severe infection, or heavy alcohol use. Long-term use may reduce vitamin B12 levels; monitoring may be needed.
Alcohol (increases lactic acidosis risk), iodinated contrast media, cimetidine, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), diuretics, corticosteroids, thyroid hormones, beta-agonists, antipsychotics (may raise glucose), insulin/sulfonylureas (increased hypoglycaemia risk when combined)
