Monograph
Insulin lispro protamine suspension 25% + Insulin lispro 75% (100 IU/mL)
Treatment of diabetes mellitus (type 1 and type 2) to improve blood glucose control when a premixed rapid-acting insulin regimen is indicated
This is a premixed insulin analogue containing rapid-acting insulin lispro and intermediate-acting insulin lispro protamine. The lispro component covers post‑meal blood sugar spikes, while the protamine component provides longer basal coverage between meals and overnight.
For subcutaneous injection using the prefilled 3 mL KwikPen as prescribed by the doctor. Typically injected within 15 minutes before a meal or immediately after a meal; dose and timing depend on blood glucose, diet and activity. Gently roll/rotate the pen to resuspend until uniformly cloudy before use; do not use if clumped or not uniformly mixed. Rotate injection sites (abdomen, thigh, upper arm, buttock); do not inject into a vein or muscle. Use a new needle each time and do not share the pen.
Hypoglycaemia (low blood sugar),injection site reactions (pain, redness, swelling),weight gain, lipodystrophy (skin thickening/pitting at injection site),rash, itching, peripheral oedema
Risk of severe hypoglycaemia—carry fast-acting glucose and monitor sugars regularly. Do not use during episodes of hypoglycaemia. Dose adjustments may be needed with changes in meals, exercise, illness, stress, or when switching from another insulin. Use caution in kidney or liver impairment; insulin requirements may change. Watch for allergic reactions; seek urgent care for breathing difficulty or widespread rash. Avoid injecting into areas of lipodystrophy. If used with pioglitazone, monitor for fluid retention/heart failure symptoms.
Other antidiabetics (sulfonylureas, GLP-1 RAs) may increase hypoglycaemia risk; corticosteroids, thiazide diuretics, thyroid hormones, sympathomimetics may raise glucose and increase insulin needs; beta-blockers can mask hypoglycaemia symptoms; alcohol may cause unpredictable glucose changes; ACE inhibitors/ARBs and salicylates may enhance glucose-lowering; MAO inhibitors may increase hypoglycaemia risk; pioglitazone may increase risk of oedema/heart failure when combined with insulin
Store unopened pens refrigerated at 2–8°C; do not freeze and keep away from the freezer compartment. Protect from light. After first use, store at room temperature (below 30°C) and use within the period stated on the pack; keep cap on and do not store with needle attached. Keep out of reach of children.
