Type 2 diabetes mellitus (to improve glycaemic control along with diet and exercise); may be used in patients with type 2 diabetes with established cardiovascular disease/heart failure or chronic kidney disease as per physician advice.
Empagliflozin is an SGLT2 inhibitor that reduces glucose reabsorption in the kidneys. It increases urinary glucose excretion, thereby lowering blood sugar and causing mild diuresis which can help reduce blood pressure and heart/renal workload.
Take orally once daily as prescribed, with or without food, preferably at the same time each day. Swallow whole with water; do not stop or change dose without medical advice.
Increased urination, thirst, genital fungal infection (vulvovaginitis/balanitis), urinary tract infection, dizziness/low blood pressure, nausea
Not for type 1 diabetes or diabetic ketoacidosis. Risk of dehydration and low blood pressure—use caution in elderly, on diuretics, or with low fluid intake. May increase risk of genital infections; maintain hygiene and seek care for pain, swelling, fever or discharge. Monitor kidney function; not recommended in severe renal impairment/dialysis (use per current label/doctor advice). Stop and seek urgent care if symptoms of ketoacidosis occur (nausea, vomiting, abdominal pain, rapid breathing, unusual fatigue) even if sugars are not very high; temporarily withhold during major surgery or prolonged fasting/acute illness. Rare risk of serious perineal infection (Fournier’s gangrene).
Diuretics (increased dehydration/hypotension), insulin or sulfonylureas (increased risk of hypoglycaemia—may need dose adjustment), antihypertensives (additive BP lowering), lithium (may reduce lithium levels), other SGLT2 inhibitors (avoid duplication).
