Treatment of type 2 diabetes mellitus to improve glycaemic control in adults, as an add-on to diet and exercise when dual therapy is appropriate.

Empagliflozin is an SGLT2 inhibitor that increases urinary glucose excretion by reducing glucose reabsorption in the kidneys. Sitagliptin is a DPP-4 inhibitor that increases incretin levels, enhancing glucose-dependent insulin release and reducing glucagon secretion, thereby lowering blood glucose.

Oral tablet. Take exactly as prescribed by the doctor, usually once daily; may be taken with or without food. Maintain adequate hydration; follow diet and exercise advice. Do not stop without medical advice.

Urinary tract infection, genital fungal infection, increased urination, thirst, dizziness, headache, nausea, diarrhoea, low blood sugar (especially with insulin/sulfonylurea)

Not for type 1 diabetes or diabetic ketoacidosis. Risk of dehydration/low blood pressure—use caution in elderly, on diuretics, or with low fluid intake. Risk of genital mycotic infections and UTIs; seek care if pain/burning urination or fever occurs. Rare but serious: ketoacidosis (even with near-normal sugars), pancreatitis (severe abdominal pain), severe joint pain, hypersensitivity reactions. Monitor kidney function; not recommended in severe renal impairment/end-stage renal disease/dialysis. Temporarily stop before major surgery or during acute illness/fasting as advised by doctor.

Insulin and sulfonylureas (increased hypoglycaemia risk), diuretics (increased dehydration/hypotension), ACE inhibitors/ARBs and other antihypertensives (additive BP lowering), rifampicin (may reduce sitagliptin levels), digoxin (sitagliptin may increase digoxin levels), lithium (SGLT2 inhibitors may affect levels—monitor)

Store below 25°C in a dry place. Protect from moisture and direct light. Keep out of reach of children.
