Treatment of type 2 diabetes mellitus in adults when diet and exercise alone are insufficient; helps control blood glucose as combination therapy.
Glimepiride is a sulfonylurea that increases insulin release from the pancreas and improves insulin action. Metformin decreases glucose production in the liver and improves insulin sensitivity, reducing intestinal glucose absorption. Together they lower fasting and post-meal blood sugar.
Take orally exactly as prescribed, usually with meals to reduce stomach upset and lower risk of hypoglycaemia. Swallow tablet whole with water; do not skip meals. Dose is individualized based on blood glucose; regular monitoring is required.

Risk of hypoglycaemia (especially if meals are missed, after exercise, or with alcohol); monitor blood glucose. Metformin may rarely cause lactic acidosis—risk increases with severe kidney impairment, dehydration, severe infection, heart/respiratory failure or excessive alcohol; assess renal function before and during therapy. Use caution in liver disease; stop temporarily before/after iodinated contrast procedures and major surgery as advised by doctor. Avoid if type 1 diabetes or diabetic ketoacidosis; seek medical help for symptoms of low sugar or lactic acidosis (rapid breathing, severe weakness, abdominal discomfort).
Alcohol, insulin/other antidiabetics (risk of hypoglycaemia), beta-blockers (may mask hypoglycaemia symptoms), corticosteroids and thiazide/loop diuretics (may raise glucose), ACE inhibitors (may enhance glucose-lowering), fluoroquinolones (glucose disturbances), rifampicin (may reduce effect), azole antifungals and macrolides (may increase sulfonylurea effect), iodinated contrast media (metformin—lactic acidosis risk), cimetidine (may increase metformin levels)
