Type 2 diabetes mellitus (in adults) when diet and exercise alone or single-agent therapy do not provide adequate glycaemic control
Glimepiride (a sulfonylurea) increases insulin release from the pancreas and improves insulin sensitivity. Metformin (a biguanide) reduces glucose production in the liver and decreases intestinal glucose absorption while improving peripheral glucose uptake.
Oral tablet; take exactly as prescribed. Usually taken once daily with or just after food (often with breakfast) to reduce stomach upset and risk of low sugar. Swallow whole (do not crush/chew if modified-release); monitor blood glucose regularly.
Hypoglycaemia, nausea, diarrhoea, abdominal pain, loss of appetite, metallic taste, headache, dizziness, weight gain
May cause hypoglycaemia—risk increases with missed meals, alcohol, heavy exercise, or with other antidiabetics; carry a glucose source. Metformin can rarely cause lactic acidosis—risk higher in kidney disease, dehydration, severe infection, heart/respiratory failure, liver disease, or heavy alcohol use; stop and seek care for rapid breathing, severe weakness, abdominal pain. Use with caution in renal impairment; kidney function should be assessed before and during treatment. Temporarily stop metformin before/after iodinated contrast procedures and during major surgery as advised. Not for type 1 diabetes or diabetic ketoacidosis.
Alcohol, insulin and other antidiabetic medicines (increased hypoglycaemia risk), beta-blockers (may mask hypoglycaemia), ACE inhibitors (may enhance glucose-lowering), corticosteroids and thiazide/loop diuretics (may raise glucose), rifampicin (may reduce sulfonylurea effect), azole antifungals and macrolides (may increase sulfonylurea effect), cimetidine (may increase metformin levels), iodinated contrast media (lactic acidosis risk), NSAIDs/salicylates (may increase hypoglycaemia)
Store below 25°C in a dry place, protected from light and moisture. Keep in original strip and out of reach of children.
