Type 2 diabetes mellitus to improve blood sugar control when diet and exercise alone are not sufficient

Glimepiride (a sulfonylurea) increases insulin release from the pancreas and improves glucose utilization. Metformin (a biguanide) reduces glucose production in the liver and improves insulin sensitivity, helping lower blood sugar.

Oral tablet. Take exactly as prescribed, usually once daily with the first main meal; swallow whole (do not crush/chew if SR/ER). Monitor blood glucose regularly and follow diet/exercise advice.

Hypoglycaemia, nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, headache, dizziness, metallic taste, weight gain

Risk of hypoglycaemia (higher with missed meals, alcohol, strenuous exercise); metformin may rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, abdominal pain, or unusual sleepiness. Use with caution in kidney or liver impairment, elderly, malnourished, adrenal/pituitary insufficiency. Temporarily stop metformin around iodinated contrast procedures or major surgery as advised. Not for type 1 diabetes or diabetic ketoacidosis. Monitor renal function, HbA1c; consider vitamin B12 monitoring with long-term metformin.

Alcohol (increases hypoglycaemia/lactic acidosis risk), insulin/other antidiabetics (additive hypoglycaemia), corticosteroids and thiazide/loop diuretics (may raise glucose), beta-blockers (may mask hypoglycaemia symptoms), ACE inhibitors (may increase hypoglycaemia), fluoroquinolones (glucose disturbances), azole antifungals/macrolides (may increase sulfonylurea effect), iodinated contrast media (lactic acidosis risk with metformin), cimetidine (increases metformin levels)

Store below 25°C in a cool, dry place. Protect from moisture and direct light. Keep out of reach of children.
