Type 2 diabetes mellitus (in adults) to improve blood sugar control when diet and exercise alone are not sufficient
Glimepiride stimulates the pancreas to release insulin. Sitagliptin (a DPP-4 inhibitor) increases incretin hormones, enhancing glucose-dependent insulin release and reducing glucagon. Metformin decreases glucose production in the liver and improves insulin sensitivity, reducing intestinal glucose absorption.
Take orally exactly as prescribed. Typically taken once daily with a main meal (often breakfast) to reduce stomach upset and risk of low sugar; swallow whole (do not crush/chew if SR/ER). Monitor blood glucose regularly and follow diet/exercise advice.
Hypoglycaemia (low blood sugar),nausea, diarrhoea, abdominal discomfort, loss of appetite, headache, dizziness, weight gain (may occur),upper respiratory tract infection
Risk of hypoglycaemia (higher with missed meals, alcohol, heavy exercise, elderly, kidney/liver disease); metformin-associated lactic acidosis is rare but serious—seek urgent care for fast breathing, severe weakness, abdominal pain, or unusual sleepiness; avoid/use with caution in renal impairment—dose adjustment and periodic kidney function tests needed; temporarily stop metformin before/after iodinated contrast procedures or major surgery as advised by doctor; sitagliptin may rarely cause pancreatitis—report severe persistent abdominal pain; use caution in hepatic impairment; not for type 1 diabetes or diabetic ketoacidosis.
Alcohol (increases hypoglycaemia and lactic acidosis risk),insulin/other antidiabetics (additive hypoglycaemia),corticosteroids and diuretics (may increase glucose),beta-blockers (mask hypoglycaemia symptoms),ACE inhibitors (may increase hypoglycaemia),iodinated contrast media (lactic acidosis risk with metformin),cimetidine (may increase metformin levels),rifampicin (may alter sitagliptin exposure),strong CYP2C9 inhibitors/inducers (may affect glimepiride levels)
Store below 25°C in a dry place, protected from light and moisture. Keep in original blister pack and out of reach of children.
