Type 2 diabetes mellitus (in adults) when blood sugar is not adequately controlled with diet, exercise and single/dual oral antidiabetic therapy
Glimepiride (a sulfonylurea) increases insulin release from the pancreas. Metformin reduces glucose production in the liver and improves insulin sensitivity. Pioglitazone (a thiazolidinedione) improves insulin sensitivity in muscle and fat by activating PPAR-γ, helping reduce blood glucose.
Oral tablet; take exactly as prescribed. Usually taken once daily with the first main meal (or as directed). Swallow whole (do not crush/chew if SR/ER); take with food to reduce stomach upset. Monitor blood glucose regularly.
Hypoglycaemia, nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, headache, dizziness, weight gain, peripheral oedema (swelling), upper respiratory tract infection
Risk of hypoglycaemia (higher with missed meals, alcohol, strenuous exercise); carry glucose. Metformin may rarely cause lactic acidosis—risk increases with kidney disease, dehydration, severe infection, alcoholism, heart/respiratory failure; stop and seek care for rapid breathing, severe weakness, abdominal pain. Pioglitazone can cause fluid retention/worsen heart failure; avoid in NYHA class III/IV and use caution in cardiac disease; monitor for weight gain/edema. Monitor kidney function and liver enzymes; stop if jaundice or significant LFT elevation. Long-term metformin may reduce vitamin B12—consider periodic monitoring. Use caution in elderly and in those with renal/hepatic impairment. Temporarily stop metformin around iodinated contrast/surgery as advised by doctor.
Alcohol (increases hypoglycaemia and lactic acidosis risk), insulin or other antidiabetics (additive hypoglycaemia/edema), beta-blockers (mask hypoglycaemia), corticosteroids and thiazide diuretics (raise glucose), rifampicin (reduces pioglitazone effect), gemfibrozil (increases pioglitazone levels), iodinated contrast media (lactic acidosis risk with metformin), cimetidine (raises metformin levels), ACE inhibitors/salicylates (may enhance hypoglycaemia)
