Type 2 diabetes mellitus in adults when blood glucose is not adequately controlled with diet/exercise and/or dual therapy; helps control fasting and post-meal blood sugar
Metformin reduces glucose production in the liver and improves insulin sensitivity. Glimepiride stimulates the pancreas to release insulin. Voglibose slows carbohydrate digestion/absorption in the intestine, reducing post-meal blood sugar spikes.
Tablet (oral). Take exactly as prescribed, usually once daily with/after meals (often with the first main meal) to reduce stomach upset and hypoglycaemia. Swallow whole with water; do not crush/chew sustained-release tablets. Follow diet and exercise advice and monitor blood glucose regularly.
Hypoglycaemia, nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, flatulence, headache, dizziness, metallic taste, weight gain, skin rash
Prescription-only (Schedule H). Risk of hypoglycaemia (higher with missed meals, alcohol, heavy exercise, elderly, kidney/liver impairment); carry sugar/glucose. Metformin may rarely cause lactic acidosis—seek urgent help for rapid breathing, severe weakness, abdominal pain, unusual drowsiness. Use with caution/avoid in severe renal impairment, severe liver disease, severe infection/dehydration, heart/respiratory failure, or alcoholism. Temporarily stop metformin around iodinated contrast studies and major surgery as advised by doctor. Monitor renal function, HbA1c, and vitamin B12 periodically. Not for type 1 diabetes or diabetic ketoacidosis.
Alcohol (increases hypoglycaemia/lactic acidosis risk), iodinated contrast media, insulin/other antidiabetics (additive hypoglycaemia), corticosteroids and diuretics (may raise glucose), beta-blockers (may mask hypoglycaemia symptoms), ACE inhibitors (may enhance glucose-lowering), fluoroquinolone antibiotics (glucose disturbances), cimetidine (may increase metformin levels), NSAIDs and other nephrotoxic drugs (increase renal risk)
