Type 2 diabetes mellitus in adults when diet and exercise along with single/dual therapy do not provide adequate glycaemic control
Glimepiride (sulfonylurea) increases insulin release from the pancreas. Metformin (biguanide) decreases glucose production in the liver and improves insulin sensitivity. Pioglitazone (thiazolidinedione) improves insulin sensitivity in muscle and fat via PPAR-γ activation, helping lower blood glucose.
Take by mouth exactly as prescribed. Usually taken once daily with the first main meal (or as directed) to reduce stomach upset and risk of low sugar. Swallow whole with water; do not skip meals; monitor blood glucose regularly.
Hypoglycaemia, nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, headache, dizziness, weight gain, peripheral oedema (swelling), upper respiratory tract infection
Risk of hypoglycaemia (higher with missed meals, alcohol, heavy exercise); metformin may rarely cause lactic acidosis—seek urgent care for fast breathing, severe weakness, abdominal pain, or unusual sleepiness; avoid/use with caution in severe kidney impairment and monitor renal function; pioglitazone can cause fluid retention and may worsen heart failure—contraindicated in NYHA III/IV heart failure; monitor for weight gain/ankle swelling; possible increased fracture risk and macular oedema with pioglitazone; use with caution in liver disease—check LFTs; temporarily stop metformin before/after iodinated contrast or major surgery as advised by doctor.
Alcohol (increases hypoglycaemia and lactic acidosis risk), insulin/other antidiabetics (additive hypoglycaemia), beta-blockers (mask hypoglycaemia symptoms), corticosteroids and thiazide/loop diuretics (raise glucose), rifampicin (reduces pioglitazone levels), gemfibrozil (increases pioglitazone levels), cimetidine (may increase metformin levels), iodinated contrast media (kidney injury/lactic acidosis risk), ACE inhibitors/ARBs (may enhance glucose-lowering)
