Glimepiride 2 mg + Metformin Hydrochloride (SR) 500 mg + Pioglitazone 15 mg
Type 2 diabetes mellitus in adults when diet and exercise alone or dual therapy is inadequate; helps control blood sugar levels and improves glycaemic control.
Glimepiride (a sulfonylurea) increases insulin release from the pancreas. Metformin reduces glucose production in the liver and improves insulin sensitivity. Pioglitazone (a thiazolidinedione) improves insulin sensitivity in muscle and fat by activating PPAR-γ, thereby reducing insulin resistance.
Tablet (oral). Take exactly as prescribed by the doctor, usually once daily with/after the first main meal to reduce stomach upset and risk of low sugar. Swallow whole; do not crush/chew the SR tablet. Monitor blood glucose regularly and follow diet/exercise advice.
Hypoglycaemia, nausea, diarrhoea, abdominal pain, metallic taste, headache, dizziness, weight gain, oedema (swelling), increased appetite
Schedule H prescription medicine. Risk of hypoglycaemia (higher with missed meals, heavy exercise, alcohol); carry a sugar source. Metformin: rare risk of lactic acidosis—higher in kidney impairment, dehydration, severe infection, hypoxia; kidney function should be checked before and during therapy; stop temporarily around iodinated contrast/surgery as advised. Pioglitazone can cause fluid retention and may worsen heart failure; use is not recommended in symptomatic heart failure. Monitor liver enzymes; avoid if active liver disease. May cause weight gain and oedema; report shortness of breath or rapid weight gain. Use caution in elderly and those with kidney/liver problems.
Alcohol (increases hypoglycaemia/lactic acidosis risk), insulin/other antidiabetics (additive hypoglycaemia), beta-blockers (may mask hypoglycaemia symptoms), corticosteroids/diuretics (may raise glucose), rifampicin (reduces pioglitazone levels), gemfibrozil (increases pioglitazone levels), iodinated contrast media (metformin—lactic acidosis risk), ACE inhibitors (may enhance glucose-lowering), NSAIDs/salicylates (may increase hypoglycaemia risk)
