Type 2 diabetes mellitus to improve blood sugar control in adults, especially when diet and exercise alone are insufficient
Glimepiride (sulfonylurea) increases insulin release from the pancreas. Metformin ER reduces glucose production in the liver and improves insulin sensitivity. Dapagliflozin (SGLT2 inhibitor) lowers blood sugar by increasing urinary glucose excretion.
Oral tablet. Take exactly as prescribed, usually once daily with food (preferably with the main meal) to reduce stomach upset. Swallow whole; do not crush/chew (ER component). Monitor blood glucose regularly.
Hypoglycaemia, dizziness, headache, nausea, diarrhoea, abdominal discomfort, loss of appetite, increased urination, thirst, genital fungal infection, urinary tract infection
Risk of hypoglycaemia (higher with missed meals, alcohol, strenuous exercise or with other antidiabetics); metformin may rarely cause lactic acidosis (risk increased in kidney/liver impairment, dehydration, heavy alcohol intake, severe infection, hypoxia)—seek urgent care for severe weakness, breathing trouble, abdominal pain. Dapagliflozin can cause dehydration/low BP, genital mycotic infections, UTIs, and rare ketoacidosis even with near-normal sugars—stop and seek care if nausea/vomiting, abdominal pain, rapid breathing occur. Avoid/use with caution in renal impairment; assess eGFR before and during therapy. Hold metformin around iodinated contrast/surgery as advised by doctor. Not for type 1 diabetes.
Insulin/other sulfonylureas (additive hypoglycaemia), alcohol (increases hypoglycaemia and lactic acidosis risk), diuretics (dehydration/hypotension), ACE inhibitors/ARBs (may enhance glucose-lowering), corticosteroids and thiazides (raise glucose), rifampicin/phenytoin (reduce sulfonylurea effect), azole antifungals/clarithromycin (may increase sulfonylurea effect), cimetidine and other renal tubular secretion inhibitors (may increase metformin levels), iodinated contrast media (lactic acidosis risk)
