Type 2 diabetes mellitus: to improve blood glucose control along with diet and exercise when dual therapy is required
Sitagliptin is a DPP-4 inhibitor that increases incretin levels (GLP-1, GIP), leading to increased insulin release and reduced glucagon in a glucose-dependent manner. Metformin decreases hepatic glucose production, reduces intestinal glucose absorption, and improves insulin sensitivity.
Oral tablet. Take exactly as prescribed, usually once or twice daily with meals to reduce stomach upset. Swallow whole with water; do not crush/chew. Monitor blood glucose regularly.
Nausea, vomiting, diarrhea, abdominal pain, loss of appetite, metallic taste, headache, upper respiratory tract infection, nasopharyngitis
Not for type 1 diabetes or diabetic ketoacidosis. Metformin may rarely cause lactic acidosis—risk increases with kidney impairment, dehydration, severe infection, alcohol use, liver disease, or hypoxia; assess renal function before and during therapy. Sitagliptin may rarely cause pancreatitis, severe joint pain, bullous pemphigoid, and hypersensitivity reactions; stop and seek care if severe abdominal pain or rash/swelling occurs. Use caution in elderly and in conditions predisposing to hypoglycaemia when combined with insulin/sulfonylureas. Withhold metformin temporarily for major surgery or iodinated contrast procedures as per clinician advice.
Alcohol (increases lactic acidosis risk), iodinated contrast media, insulin or sulfonylureas (increased hypoglycaemia risk), cationic drugs cleared by renal tubular secretion (e.g., cimetidine) may increase metformin levels, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide) may increase metabolic acidosis risk, digoxin (sitagliptin may slightly increase levels)
