Type 2 diabetes mellitus to improve blood glucose control in adults, as an adjunct to diet and exercise
Vildagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), enhancing insulin release and reducing glucagon in a glucose-dependent manner. Metformin reduces hepatic glucose production and improves insulin sensitivity, lowering blood glucose levels.
Oral tablet. Take as prescribed, usually with meals to reduce stomach upset. Swallow whole (do not crush/chew if SR/ER). Maintain diet and exercise; monitor blood sugar regularly.
Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, headache, dizziness, upper respiratory tract infection, joint pain, low blood sugar (especially with other antidiabetics), metallic taste
Risk of lactic acidosis with metformin (seek urgent care for rapid breathing, severe weakness, muscle pain, drowsiness). Avoid/use with caution in kidney impairment; check eGFR before and during therapy. Use caution in liver disease, alcohol use, dehydration, severe infection, hypoxia, or heart failure. Withhold metformin before/after iodinated contrast procedures and major surgery as advised. Monitor for pancreatitis symptoms (severe abdominal pain), hepatitis/jaundice, and vitamin B12 deficiency with long-term use.
Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cimetidine, diuretics, corticosteroids, thyroid hormones, sympathomimetics, other antidiabetics/insulin or sulfonylureas (increased hypoglycaemia risk), ACE inhibitors (may enhance glucose-lowering)
