Type 2 diabetes mellitus (adult) to improve blood sugar control when diet and exercise alone or single therapy is inadequate
Glimepiride is a sulfonylurea that increases insulin release from pancreatic beta cells and improves glucose utilization. Metformin reduces glucose production in the liver and improves insulin sensitivity, lowering blood sugar without usually causing hypoglycaemia when used alone.
Oral tablet; take exactly as prescribed. Usually taken once daily with a main meal (often breakfast or dinner). Swallow whole with water; do not crush/chew (especially if sustained/prolonged release). Monitor blood glucose regularly; follow diet and exercise advice.
Hypoglycaemia, nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, headache, dizziness, weight gain, metallic taste
Risk of hypoglycaemia (more likely with missed meals, excess exercise, alcohol, elderly, kidney/liver impairment); carry a quick source of sugar. Metformin can rarely cause lactic acidosis—seek urgent help for rapid breathing, severe weakness, muscle pain, abdominal discomfort, or unusual sleepiness. Use caution/avoid in severe renal impairment; dose adjustment and renal function monitoring required. Temporarily stop metformin before/at iodinated contrast imaging and major surgery as advised by doctor. Use caution in liver disease, heart failure, dehydration, severe infection, or alcohol misuse. Do not use in type 1 diabetes or diabetic ketoacidosis.
Alcohol (increases risk of hypoglycaemia and lactic acidosis), insulin/other antidiabetics (additive hypoglycaemia), beta-blockers (mask hypoglycaemia symptoms), corticosteroids/thiazide diuretics/sympathomimetics (raise glucose), ACE inhibitors (may increase hypoglycaemia), fluoroquinolones (glucose disturbances), cimetidine (increases metformin levels), iodinated contrast media (risk of lactic acidosis), NSAIDs and sulfonamides (may increase hypoglycaemia), rifampicin (may reduce effect)
