Gastroesophageal reflux disease (GERD), acidity/heartburn, gastritis, peptic ulcer disease (adjunct), nausea and vomiting associated with acid reflux or functional dyspepsia

How it works

Pantoprazole is a proton pump inhibitor that reduces stomach acid by blocking the H+/K+ ATPase pump in gastric parietal cells. Domperidone is a prokinetic/antiemetic that blocks dopamine D2 receptors (mainly peripherally), improving gastric emptying and reducing nausea.

Directions for use

Take as directed by physician. Generally, 1 capsule once daily 30 minutes before food (preferably before breakfast); swallow whole, do not crush or chew. If prescribed twice daily, take before meals.

Side effects

Headache, diarrhea, abdominal pain, nausea, flatulence, dry mouth, dizziness, constipation, increased prolactin (breast tenderness/galactorrhea), fatigue

Warnings & precautions

Prescription medicine. Domperidone may increase risk of QT prolongation/serious heart rhythm problems—avoid in patients with known QT prolongation, significant heart disease, or with electrolyte disturbances. Use with caution in liver impairment; avoid in moderate to severe hepatic impairment. Long-term PPI use may be associated with vitamin B12 deficiency, low magnesium, bone fracture risk and Clostridioides difficile diarrhea; use lowest effective dose. Seek medical help for black stools, vomiting blood, unexplained weight loss, persistent vomiting or chest pain. Not recommended for children unless advised by doctor.

Drug interactions

Domperidone: avoid with strong CYP3A4 inhibitors and QT-prolonging drugs (e.g., ketoconazole/itraconazole, erythromycin/clarithromycin, ritonavir, certain antipsychotics/antiarrhythmics). Pantoprazole: may reduce absorption of drugs needing acidic pH (e.g., ketoconazole, atazanavir); may increase INR with warfarin (monitor), may affect methotrexate (high-dose) levels. Antacids can be taken but keep timing as advised.
