Esomeprazole is a proton pump inhibitor that reduces stomach acid by blocking the H+/K+ ATPase pump in gastric parietal cells. Domperidone is a dopamine (D2) receptor antagonist that increases gastric motility and helps relieve nausea and reflux by improving stomach emptying.
Capsule (SR): take exactly as prescribed, usually once daily 30 minutes before food (preferably before breakfast). Swallow whole; do not crush, chew or open the capsule. If a dose is missed, take it when remembered unless it is near the next dose.
Headache, diarrhea, abdominal pain, nausea, flatulence, dry mouth, dizziness, constipation
Do not use if allergic to esomeprazole/domperidone or other PPIs. Domperidone may increase risk of QT prolongation/serious heart rhythm problems—use caution in heart disease, electrolyte imbalance, and in older patients; avoid with known prolonged QT. Prolonged PPI use may be associated with low magnesium, vitamin B12 deficiency, fractures, and C. difficile-associated diarrhea; seek care for severe/persistent diarrhea. May mask symptoms of gastric cancer—consult doctor for alarm symptoms (weight loss, vomiting blood, black stools, difficulty swallowing). Use caution in severe liver disease; kidney disease: consult doctor.
QT-prolonging drugs (e.g., amiodarone, sotalol, certain antipsychotics/antidepressants), strong CYP3A4 inhibitors (e.g., ketoconazole/itraconazole, erythromycin/clarithromycin, ritonavir) increase domperidone levels and arrhythmia risk; warfarin (monitor INR), clopidogrel (reduced activation possible with some PPIs), digoxin, methotrexate (high dose), atazanavir/nelfinavir (reduced absorption with PPIs), iron salts and other drugs needing acidic pH (reduced absorption)
